I agree to the below.

I/we request the approval of the Owners Corporation to keep the above described pet, subject to the following agreed conditions:
1. The approval is for the pet named above and not for any other pet.
2. The pet will stay within the confines of my/our lot except when entering and leaving the strata scheme.
3. When entering and leaving the strata scheme, the pet will be carried or kept on a lead by a responsible person who will control the pet at all times when on the common property of the strata scheme. That responsible person will use sound judgement as to whether a cat and dog should travel together in the strata’s small lift.
4. I/We will maintain a high standard of preventative health care, including treatment for flees, lice, worms and other parasites.
5. It will be my/our responsibility as pet owners to monitor the noise from the pet, ensuring it does not unreasonably cause annoyance or disturbance to neighbours.
6. I/We will ensure that the pet does not create any nuisance or hazard to the owners or occupiers of any other lot or unreasonably interfere with the use or enjoyment of any other lot or the common property.
7. I/We will train the pet to soil or toilet within our lot and not on common property and in the event of an accident I/We will immediately remove, cleaned, repair and appropriately dispose of any soiling which may occur on a lot or on common property.
8. I/We will ensure a high standard of cleanliness and sanitation and at all times any waste will be disposed of promptly and hygienically so as to ensure no unpleasant odour or other nuisance, hazard, or unreasonable interference with the use and enjoyment of any other lot or common property.
9. I/We will accept full responsibility for and indemnify the Owners Corporation against all claims for loss of or damage to property or death of or injury to any person arising, whether directly or indirectly, from, or as a result of, any action of the pet.
10. I/We acknowledge that in the event of a breach of this agreement the Owners Corporation may withdraw any approval it has given me/us for the keeping of the pet.
11. I/We understand that dogs of a prohibited breed or declared restricted or dangerous must not be kept or brought onto a lot or common property.
12. I/We have attached a report from a Veterinarian on the suitability of keeping the pet in my/our lot and advising of any measures to be taken to ensure the health, safety and welfare of the pet and owners and occupiers of other lots and I/we will ensure that those measures are taken.
13. I/We will comply with such other conditions as the Owners Corporation acting reasonably may impose on the approval.
14. If applicable, provide written evidence that the animal is an Assistance animal under sect 9(2) of the disability Act.
